Abstract: Objective To analyze and explore the safety and effectiveness of temporary with-drawal of immunosuppressants (IMS) because of severe infection after liver transplantation. Methods The clinical data of 11 cases having temporary withdrawal of IMS for 13 times after orthotopic livertransplantation between October 2003 and March 2006 were analyzed retrospectively. Results It was amedian of 13. 5 days (range 5-26 days) with IMS discontinuation. Before May 2005, 6 patientsdiscontinuedlMS (<10 days) for 8 times. Only one patient was healed, and the remaining 5 patientswere died because of multiple organ failure. After May 2005, 3 of 5 patients who had withdrawal ofIMS for more than 20 days were healed, and the remaining 2 patients with infection because ofpulmonary embolism or hepatic artery embolism were dead. Only one patient developed mild acuterejection, and treated with reinstitution of the former dosage of FK506. Conclusion It is safe totemporarily withdraw IMS because of severe infection after liver transplantation. Even though theacute rejection occurs, it is easy to be reversed by reinstitution of FK506 in most cases and treatmentwith steroid boluses in very few.
