Abstract: Objective To discuss the feasibility of microsurgery for petroclival meningiomas through tempoccipital transtentorail approach.Methods The retrospective analysis of eighteen cages of petroclival meningiomas Was performed.All the tumors were resected through tempoccipital transtentorail approach.All patients were scanned by MRI before operations.Results The size, of tumors:four cases<3.0 cm.five cases 3.0-4.5 cm,nine csses>4.5 cm.One patient had the biggest tumor(7.6cm×7cm ×7cm).Total resection was,achieved in eight cases,subtotal resection in nine cases.There were two cases with paralysis,one case with hemifacial palsy,and one cage with heating loss.Cerebrospinal fluid leakage and aphasia were not observed.One patient died after operation.Conclusion Tempoccipital transtentorail approach could be useful for resection of petroclival meningiomas.Surgeon could get a good view involving the ares of cavernous sinus,clivus and cerebellopontine angle.
