New MRA postprocessing techniques in detection of intracranial aneurysms and treatment significance

Abstract: 目的利用最新磁共振血管成像后处理技术诊断和观察动脉瘤。方法经手术和血管造影证实脑动脉瘤38例共43个,所有病例都进行脑部磁共振成像和磁共振血管成像,血管成像序列包括三维时间飞跃法(3D time of flight,3D TOF)或合并零位差值技术(zerofill interpolation processing,ZIP),后处理技术包括常规技术即最大强度投影( maximum intensity projection, MIP)、多平面断层血管重建(multiplanar tomographic vascular imaging, MTVR)和三维导航血管表面重建(3D navigator vascular imaging,3D NVI)技术。结果对小动脉瘤的检出率,MTVR(100.0%)和3D NVI(94.4%)明显高于MIP(66.7%)(x2=15.289,P<0.01),对动脉瘤颈部的显示率,MTVR(90.7%)和3D NVI(90.7%)明显高于MIP(72.1%)和原始图像(79.1%)(x2=4.913,P<0.01)。仿真内窥镜成像可显示血管和动脉瘤的内部结构、动脉瘤的异常开口和角度,并进行动态观察。结论 MTVR与3D NVI后处理技术对诊断和制订动脉瘤的治疗计划有着重要指导意义。
