Abstract： Objective To explore the predictive value of fibulin 5(FBLN5),stromal cell-derived factor-1α(SDF-1α)and P-glycoprotein(P-gp)in surgically removed tissues of non-small cell lung cancer(NSCLC)for tumor-free survival of the patients.Methods A total of seven hundred and ninety-four patients with NSCLC who underwent surgical resection in the hospital between January and December 2021 were selected as re-search subjects.The normal tissues on incised margin and cancer tissues were collected after surgery.The ex-pressions of FBLN5,SDF-1α and P-gp were detected by real-time quantitative PCR(RT-PCR)and immuno-histochemistry.The positive expressions of FBLN5,SDF-1α and P-gp in normal resection margin tissues and cancer tissues,and among patients with different clinicopathological characteristics were compared.The influ-encing factors of tumor-free survival in NSCLC patients were analyzed by Logistic regression analysis.Results The positive expression rate of FBLN5 in normal resection margin tissues was higher than that in cancer tissues,while positive expression rates of SDF-1α and P-GP were lower than those in cancer tissues(P＜0.05).There were significant differences in positive expression rates of FBLN5,SDF-1α and P-GP among NSCLC patients with different clinical staging and differentiation degree(P＜0.05).The results of 1-year fol-low-up showed that in 794 patients,there were 573 survived cases,and 1-year tumor-free survival rate was 72.17％.There were significant differences in 1-year tumor-free survival rate among NSCLC patients with dif-ferent clinical staging and expressions of FBLN5,SDF-1α and P-GP(P＜0.05).The results of multivariate Logistic regression analysis showed that FBLN5,SDF-1α and P-GP were influencing factors of tumor-free sur-vival in NSCLC patients(P＜0.05).Kappa values of FBLN5,SDF-1α and P-GP for predicting tumor-free sur-vival were 0.450,0.287 and 0.395,respectively.Conclusion The expression of FBLN5 is down-regulated,while expressions of SDF-1α and P-gp are up-regulated in NSCLC tissues.FBLN5,SDF-1α and P-gp are all in-fluencing factors of tumor-free survival in NSCLC patients.Clinically,the three indexes can be applied to pre-dict tumor-free survival.
