Experimental study on prefabricated neurotrophic vascular skin flap

Abstract: Objective The feasibility of prefabricated neurotrophic vascular skin flap was studied through ani-mal experiments,aiming to provide experimental basis for the clinical application.Methods A total of forty New Zealand white rabbits weighing about 1.5 kg were used in the experiment.The prefabricated neurotrophic vascular skin flap model was firstly established:a 3~4 cm long section of lateral femoral cutaneous nerve was separated from the lateral femoral,and the distal end of the nerve was cut off.The proximal end of the nerve was rotated 160° and then trans-positioned and implanted into a skin flap made on the dorsal side of the pelvis The cutaneous nerve was sutured and fixed in the fascial layer of the skin flap with 8-0 nylon thread,and the skin flap was sutured in situ.Then,40 rabbits were divided into the experimental group and the control group,each with 20 animals.In the experimental group,3 weeks after the establishment of the model,the flap was lif-ted free,only the transposed cutaneous nerve was retained at the pedicle of the flap,and the flap was cut off and re-sutured in situ.In the control group,the translocated cutaneous nerve was not retained when the flap was cut off and re-sutured in situ 3 weeks after the establishment of the model.The red and white reaction,temperature,color,blister and necrotic area of skin flap were observed after operation.Four weeks after the operation,the skin flaps were cut off,and the survival area and histological results of the flaps were analyzed statistically.Results The flap survival rate in the experimental group was significantly higher than that of the control group(P<0.01).The number of blood vessels of the flap in this group was also significantly greater than that of the control group(P<0.01).Conclusion It is feasible to apply prefabricated neurotrophic vascu-lar skin flap in experimental rabbits.Neurotrophic blood vessels played an important role in the survival of the neurotrophic vascular skin flaps.Three weeks after the transposition and the implantation of cutaneous nerve,the neurotrophic blood vessels of the cutaneous nerve could nourish the peripheral skin.Clinically,according to this experiment,the prefabricated neurotrophic vascular flap can be designed.For example,transfers the cuta-neous nerve of the more tense area of the skin to the more relaxed area of the skin.After the prefabrication,the skin of the relaxed area can be used as the supply area of the flap.With further experimental studies,further clinical applications of this prefabricated skin flap can be explored.
