Abstract: To the editor:A 31-year-old primigravida was referred to our hospital at 20 weeks of gestation.Ultrasound examinations were performed using a Prosound αtl0 (Aloka Co,Ltd,Tokyo,Japan) with a 2-6 MHz transabdominal probe.The initial sonographic findings were suggestive of a hyperechoic,homogeneous,and well-defined mass on the right lower lung,18 mm at its maximum diameter.There were no other abnormalities.We considered it to be fetal congenital cystic adenomatoid malformation (CCAM).Follow-up ultrasonography at 25 weeks of gestation showed an irregular mass of 29 mm×22 mm×23 mm within the pericardium,partially protruding into the mediastinum.Sonographic findings show the mass pulsates with the heart beat,but the heart was normal.The thickness of the fetal pericardial fluid areas was about 5.2 mm (Figure 1A and B).Differential diagnosis includes extralobar pulmonary sequestration (EPS),CCAM,and cardiac tumor.
