Abstract: To the editor:Bricker ileal conduit is one kind of surgical modality for urinary diversion.Several anastomoses must be accomplished, mainly including ureteroileostomy and the formation of ileal conduit, which increases the possibilities of urinary leakage.Diagnosis and treatment of potential urinary leaks is a real challenge.1 In this article,we report a complex case of persistent uro-cutaneous fistula after Bricker operation.A 71-year-old man underwent radical cystectomy,Bricker operation and Hartmann operation simultaneously. On day 4 after operation, loose stools polluted the puncture region. The draining liquid became muddy and the output began increasing.Biochemical test showed that creatinine was 4601 μmol/L in the draining liquid. The left ureteral Mono-J stent fell off on the day 12.Regular methods failed to find the urinary leak.Specialists of Interventional Technology Department suggested improving the method of fistulography.(1) A guide wire with flexible tip was inserted into drainage tube and preceded gently. It passed through the fistula and further went into the ileal conduit.
