Abstract: Objective:To analyze the influencing factors of basilar aneurysm rupture based on computed tomography angiography (CTA).Methods:This study was a retrospective study, a total of 196 patients with basilar aneurysms admitted to department of Imaging in The Fifth Clinical Medical College of Henan University of Traditional Chinese Medicine (Zhengzhou People′s Hospital) were enrolled between June 2020 and April 2021.The clinical data of patients were collected, there were 101 males and 95 females, with the age was (61.75±8.26) years old, ranging from 40 to 79 years old.All patients underwent CTA.The relationship between morphological parameters of aneurysms [sites and aneurysm neck of ruptured and unruptured aneurysms, parent vessel average diameter (Dv), flow angle (θF), parent-daughter angle (θP), size of ruptured aneurysms, Tumor body depth and tumor neck width aspect ratio (AR), aneurysm depth to parent artery diameter size ratio (SR), area ratio]and basilar aneurysm rupture was analyzed by logistic regression analysis. Results:Of the 196 patients, there were 127 cases with aneurysm rupture and 69 cases without rupture. The difference in sites, aneurysm neck, Dv, θF and θP between ruptured and unruptured aneurysms was not statistically significant ( P>0.05). The size [(5.52±1.01) mm], AR (1.90±0.58), SR (2.26±0.65) and area ratio (1.40±0.41) in ruptured aneurysms were all higher than those in unruptured aneurysms[(4.17±1.14)mm, (1.28±0.37), (1.51±0.39), (1.22±0.35)], and the differences were statistically significant ( P<0.05). The results of logistic multivariate linear regression analysis showed that increased aneurysm size, AR, SR and area ratio were independent risk factors of basilar aneurysm rupture, and the differences were statistically significant ( P<0.05). Conclusions:In the parameters measured by CTA, long aneurysm diameter, high area ratio, high AR and high SR were independent risk factors of aneurysm rupture.
