Abstract: Objective To discuss the classification and microsurgical resection of dumbbell spinal tumors with beth intracanalicular and extracanalicular extension. This clinical data was reviewed to analyze microsurgical approaches,technique and postoperative management. Methods Fifty-six patients underwent microsurgical resection of dumbbell spinal tumors. The tumors comprised mostly Schwannoma (84%,47/56) and meningioma (9%,5/56) in our group. According to the results of MRI and the volume of dumbbell spinal tumors,we divided to the dumbbell spinal tumors into four types. Results Total tumor removal was achieved in 51 patients (91%),subtotal in 3 patients,and partial in 2 patient. No patients died after operation and clinical symptoms were improved in all patients except 2 metastatic tumor.Conclusions It is important to classify the dumbbell spinal tumor into the reasonable type for the purpose of choose a appropriate surgical approach. Microsurgical techniques can raise the rate of total tumor removal and reduce disability of patients.
