Experimental study of orthotopic canine heart allo-transplantation after 3-hour preservation by hypothermic immersion storage

Abstract: Objective To investigate the protective effect of myocardial preservation with St Thomas extracellular-type solutions,Collins introcellular-type solutions and combination of this two solutions under hypothermic storage.Methods The following three groups were set up:group 1,the hearts being arrested with St Thomas solutions and flushed with and stored in St Thomas solutions;group 2,the hearts being arrested with Collins solution and then flushed with and stored in Collins solutions;group 3,the hearts being arrested with St Thomas solutions and then flushed with and stored in Collins solutions for 3 h.The orthotopic allotransplations were performed on the preserved hearts.Results The myocardial weight gain was greater in group 1 than in group 2 and group 3 at the end of 3 h.The mean values of post operatively released creatine kinase-MB (CK--MB) was much less in group 3 than in group 1 and in group 2.Electron microscopy showed that the cell membrane remained intact,there was minimal change in nuclei and myofibribles in all three groups.The mitochondria was preserved best in the group 3,the capillary endothelial base membrane was not intact in the group 2.Better recovery of cardiac function and less rate of intraoperative defibrillation was present in the group 3.Conclusions The donor heart preserved with immersion storage for 3 h is simple and effective.To achieve a better result,the heart should be first arrested with the extracellular-type solutions and then flushed with and stored in intracellular-type solutions for cold immersion storage.
