Abstract： Objective To explore the management models for community-based elderly patients with hypertension.Methods One hundred and sixty eight elderly patients with hypertension were classified into grades Ⅰ～Ⅳ mainly base on the severity,hypertension target organ and/or function impairment.Results Treatment of hypertension was 100% and total control rate (112 cases) is 67.1%.Control rate of grade Ⅰ/Ⅱ,Ⅲ and Ⅳ was 87.5%,71.7%,32.5% respectively.Conclnsion The management models for Community-baaed elderly patients at different levels are well-established.
