Abstract: Objective To observe the changes of glomerular and renal tubular functions in type 2 Diabetes and investigate the factors influencing renal functions. Methods The glomerular?proximal and distal tubular parameters were examined in urinary samples from 40 out patients with type 2 Diabetes by the method of Array Protein System. The urinary content of albumin?transferrin as markers of the glomerular damage; α 1 microglobulin ? N acetyl β D glucosaminidase as parameters of proximal tubule and urine condense test as parameter of distal tubule were examined. Meanwhile, the correlation between duration of disease,age, metabolic index including GHbA1c , TG, Cho and renal variables such as ALB, TRF, NAG, α 1 MG were judged by pluralistic analysis. Results An increasing urinary excretion for ALB, TRF, NAG, α 1 MG occurred during the early stage of diabetes, at the rates of 22.5%,10%,30% and 37.5% respectively. The duration of disease, plasma glucose and lipid level correlated with partial renal functions in diabetes. Conclusions These findings suggested that proximal tubular as well as glomerular dysfunction precede clinical diabetic nephropathy. It will be beneficial to prevent from DN by correcting the metabolic parameters early.
