Abstract: 目的:建立一种髓内循环破坏致骨内高压形成的动物模型的可靠方法并观察钻孔减压的影响.方法:采取兔单侧股骨髓腔内灌注骨水泥,破坏股骨髓内循环致骨内高压形成.分别测量钻孔减压组和未钻孔减压组不同时段髓内压的改变,通过HE染色光镜、电子显微镜以及免疫组化等方法,观察不同时段远端关节软骨的形态和代谢改变.结果:破坏髓内循环后骨内高压形成,在未钻孔减压组骨内高压长时间存在,随时间推移压力呈下降趋势,但32 wk后仍高于正常,远端关节软骨出现骨性关节炎代谢表现;在钻孔减压组,一次钻孔后,骨内压就可恢复正常水平,未见关节软骨损害出现.结论:所建立的髓内循环破坏致骨内高压形成的动物模型可靠,持续时间较长且远端关节软骨出现相应损害;钻孔减压对治疗骨内高压简单有效.
