Preventive and therapeutic effects of hyperbaric oxygen and hypobaric hypoxia preconditioning on cerebral infarction after permanent middle cerebral artery occlusion in rabbits and its mechanism

Abstract: 目的:观察高压氧预处理(HBOP)和低压缺氧预处理(HHP)对兔大脑中动脉闭塞(MCAO)后局灶性脑梗死程度及周围脑组织氧分压(PbtO2)的影响.方法:雄性家兔72只随机分为单纯MCAO组,HBOP+MCAO组(100%O2,2.5ATA,1 h/d,5 d)和HHP+MCAO组(4000 m,61.6 kPa,3d),每组24只,采用改良Bederson法建立家兔MCAO模型,分别于MCAO前、MCAO后1,3和10 d观察脑MRI信号强度、脑梗塞灶体积及梗塞灶边缘PbtO2.结果:MRI结果显示MCAO后3,10 d时HBOP+MCAO组和HHP+MCAO组缺血区异常信号范围较单纯MCAO组明显缩小,HBOP+MCAO组缺血区标准化信号强度(SIR)于3,10 d时分别为1.62±0.09,1.17±0.10;HHP+MCAO组缺血区SIR于3,10 d时分别为1.64±0.06,1.20±0.09;均低于单纯MCAO组的1.83±0.07,1.36±0.06(P<0.05).TTC染色显示的梗塞灶体积在3和10 d时HBOP+MCAO组和HHP+MCAO组小于单纯MCAO组;HBOP+MCAO组家兔梗塞灶边缘PbtO2于1,3和10 d时分别为(8.27±0.58),(11.92±1.27)和(16.12±1.42)mmHg;HHP+MCAO组家兔梗塞灶边缘PbtO2分别为(8.46±0.90),(10.77±1.19)和(15.33±1.29)mmHg,均高于单纯MCAO组的(7.03±1.06),(8.43±0.86)和(11.83±1.28)mmHg(P<0.05).结论:HBOP及HHP对家兔MCAO后局灶性脑缺血具有一定的防治作用.
